Jordan Hellman, JD, NRPF, is the Chief Development Officer of Care Partners, where he leads strategic growth, market expansion, and payer and provider partnerships across California. With nearly 15 years of healthcare leadership experience and a legal background, Jordan has helped develop and scale innovative healthcare programs serving seniors, individuals with disabilities, and high-risk Medi-Cal populations through home care, Enhanced Care Management (ECM), and Community Supports. Throughout his career, he has built and led multi-regional growth initiatives, developed high-performing teams, and partnered with health plans, hospitals, skilled nursing facilities, physician groups, and community-based organizations to improve care transitions and expand access to community-based care. Jordan is a National Readmission Prevention Fellow (NRPF), serves on the Board of Directors for the Southern California Chapter of the American Case Management Association (ACMA), and frequently speaks on care transitions, post-acute care, and value-based healthcare delivery.
