Collaboration to launch innovative model of care for home and community care patients

TORONTO, ON – November 30, 2015: AlayaCare, providers of next generation home care software solutions, announced today that it has been selected as an award recipient of the AdvancingHealth Program.

Along with its partners, AlayaCare will launch virtual care technology that enables home care providers to administer care via remote patient monitoring (RPM), telehealth and video conferencing. The AdvancingHealth Program, managed by Ontario Centres of Excellence (OCE), is designed to facilitate partnerships between Ontario public healthcare organizations, companies and academic institutions to increase innovation in Ontario’s public healthcare sector.

“Our AdvancingHealth program is providing an unprecedented opportunity for the public and private sectors to work collaboratively to pilot innovative products that will improve the patient experience in Ontario’s healthcare system,” says Dr. Tom Corr, President and CEO of Ontario Centres of Excellence. “Ontario has a wealth of tech innovations that are ideally suited to modernizing the healthcare sector.”

AlayaCare, along with Southlake Regional Health Center (Southlake), CBI Health Group, Professor Carolyn McGregor at the University of Ontario Institute of Technology (UOIT), and the Centre for Excellence in Economic Analysis will seek to reduce hospital admissions and emergency department visits for chronically ill patients.

They are one of five teams awarded up to $250,000 to improve patient outcomes and enhance patient experiences through the use of innovative technologies.

“AlayaCare is very encouraged to see Ontario’s investment in innovative care delivery models through programs like OCE AdvancingHealth,” said Adrian Schauer, CEO of AlayaCare. “We are thrilled to partner with public sector innovation leaders like the Southlake Regional Health Center and forward thinking providers like CBI Health Group.”

“Innovative thinking is not new to us here at Southlake. We’re passionate about discovering and developing new innovative processes, software solutions and devices that will result in better ways to deliver healthcare services,” says Dr. Dave Williams, President and CEO, Southlake Regional Health Centre.

“It was a natural fit for us to participate in this program. Our hope is that we can look to reduce hospital admissions for patients enrolled in the program by at least 50%, demonstrate a positive return on investment from a health economics point-of-view, and demonstrate that a public-private partnership (PPP) between hospitals and private community care providers is an efficient service delivery model. This is a great proof point for CreateIT Now at Southlake — our innovation incubator — and its role in spurring innovation.”

AlayaCare’s role within the project is to provide innovative technologies such as a mobile care app, Telehealth Dashboard, Family Portal, RPM software, point of care clinical documentation software, an electronic visit verification system and Machine Learning algorithm-based clinical support tools, which allow for better coordination and quality of care across the healthcare continuum.

The key deliverables of the project impacting the Ontario healthcare system are:

  • Reduced hospital re-admissions and Emergency Department (ED) visits
  • Positive health economics (i.e. reduced health system utilization)
  • Improved patient satisfaction
  • Positive experience for the family health teams at all hospital partners as evidenced by a desire to see the program continued and expanded

“With our collaboration to date, we’ve already seen the theoretical potential for remote patient monitoring combined with machine learning algorithms to reduce negative health events,” said Rudy Mancini, Senior Director, National Partnerships and Healthcare Innovation, CBI Health Group. “Now, we are ready to learn more in the field with this multi partner partnership – the potential to improve healthcare system efficiency and, importantly, patient safety.”
